What Is an AI Scribe? How AI Medical Scribes Work, What They Cost, and What to Ask

An AI scribe listens to a patient visit and drafts the clinical note, so the clinician can look at the patient instead of the keyboard. Here’s how they work, what they cost in 2026, what the evidence says, and what to check before you sign up.

What an AI scribe does

Most AI scribes today are "ambient": with the patient's knowledge, they listen to the conversation during a visit, transcribe it, and draft a structured clinical note, such as a SOAP note, in the clinician's preferred format. The clinician reviews, edits, and signs the note. Many scribes can also send the note into the electronic health record and suggest billing codes.

Do AI scribes actually save time?

The best evidence so far comes from Kaiser Permanente. Between October 2023 and December 2024, 7,260 of its physicians used ambient AI scribes across about 2.5 million patient visits. The group estimated the tools saved 15,791 hours of documentation, the equivalent of 1,794 eight-hour workdays. In surveys, 84% of physicians said it improved how they connect with patients, and 82% said it raised their job satisfaction (American Medical Association).

What to ask before you choose one

  • Will they sign a BAA? Any vendor handling patient information must sign a HIPAA Business Associate Agreement.
  • How is consent handled? Patients should know the visit is being recorded, and some states require consent from everyone being recorded. Build that into check-in.
  • How accurate is it in your specialty? Scribes can mishear medications, miss details, or add things that weren't said. The clinician must review every note before signing.
  • Does it connect to your EHR? Copying and pasting notes eats into the time saved.
  • What happens to the audio? Ask how long recordings are kept, where, and whether they're used to train the vendor's models.
  • How is it priced? Flat monthly, per note, or per visit, and what counts toward limits.

AI scribe vs AI receptionist

An AI scribe handles documentation during the visit. An AI receptionist handles what happens before and after: answering calls, scheduling, reminders, and routing refill requests. For many small practices, the phones cost as much staff time as the charting. See how an AI receptionist for medical offices works.

Frequently asked questions

What is an AI scribe?

Software that listens to a patient visit, with consent, and drafts the clinical note for the clinician to review and sign. Most are called ambient AI scribes because they work in the background of the conversation.

How much does an AI medical scribe cost?

Small-practice tools publish plans from about $40 to $120 per clinician per month. Enterprise scribes cost several hundred dollars per clinician per month and up, with pricing negotiated by larger organizations.

Are AI scribes HIPAA compliant?

Compliance depends on the vendor and your setup. Choose a vendor that signs a Business Associate Agreement, understand how audio and notes are stored, and follow your own privacy policies.

Do patients have to consent to an AI scribe?

Patients should always be told, and some states require consent from everyone being recorded. Check the rules where you practice and build consent into check-in.

How accurate are AI scribes?

Generally good, but not perfect. They can mishear medications, miss details, or add things that weren’t said, so the clinician must review every note before signing.

Do AI scribes save time?

Yes, in the largest study so far. Kaiser Permanente estimated its physicians saved 15,791 hours of documentation across about 2.5 million visits over roughly 15 months.

Also losing time on the phones?

An AI receptionist handles scheduling, reminders, and routine calls for medical practices, built only on vendors that sign a BAA. The first consult is free.
